First things first, I know we are talking about GTA, but listen, if you have not played Scarface - The World is Yours, or seen the original film, then you are seriously missing out. Even 4 years after its release, the game (released in 2006), is still very playable and holds a lot of enjoyment.

Get yourself a copy of Scarface, especially if you've not played it and even if you've already played it, (get it out and play it again), you will have a new view of the game, a new perspective. Just keep thinking, this feature or that feature would be great in GTA!
See past the better than Saints Row style brick-like car handling, (I think its a 1000 times better than SR). If you've not played it, do yourself a favor and watch the film first, because the game takes up from where the film left off.
There are loads of great features that I know the GTA team could do a million times better. But the main thing I want in my perfect GTA game is the negotiation meter found in Scarface.
A red line spins clockwise round the dollar sign, when you hold a button on your controller. If you keep holding the button, the clockwise motion continues to repeat over and over. If you stop the progress of the movement by letting go of the button, in the first quarter, (between 12 and 3 on a clock-face), then you possibly will fail or get the lowest amount. The best you can do, is just before the 12 O'clock position.
You can talk your way out of arrest by police, negotiate the amount of cash you get or pay in drug deals both buying and selling, Use it to lower the interest rate you pay when depositing / laundering money at the bank.
We Want Realism!
You get guns in the boot of your own car. Where 'does' Niko put that rocket launcher? Some of the obvious things that stand out to me is that this fantastic game of GTA, has gone into amazing detail with the vehicle handling physics, yet still has you pulling a rocket launcher out of you back pocket.

You Got Balls Man!
You get 'Balls' points added to your score and earn reputation. You see, if you think you can take on a gang-war or drugs-war, without any back-up then it shows you have confident skills and 'balls'. These 'balls' points could be used to make opposing gangs afraid of you or alternately as you start to do risky things like that, the opposing gangs actually see you as more of a threat and start to come after you.
Its a bit like your police visibility, the Scarface game has with the police, (which is another good thing to add into GTA), where your actions turn up your police visibility, till they will almost chase you on sight! Which is a very realistic approach. Then if you can stop and attempt a bribe, this uses the negotiation reticule again. You can pay off the cops, this would also add 'balls' points to your score.
Watch this video of the opening training in Scarface and look out for the 'balls' points being accumulated as Tony shoots targets and again as he 'taunts' them.
Taunt Me You Tease!
You can 'taunt' your enemies between shooting them and this adds to your 'balls' points also. Then best part is where you go into a 'blind rage', when you have saved up enough 'balls' points. You release your rage, going into a white-out kind of slow motion, akin to your adrenalin rush and increased reaction time. This is done via a very reasonable limited time slow motion view.
Sharks!
Another good touch is sharks in the water! Now, I actually like the swimming part, so I don't want them everywhere! That would be like not being able to swim in the first-place and we all know how annoying that was from previous GTA's. I think there should be a zoo where you can dispose of bodies in the shark enclosure and maybe we could have a crocodile-pen too! If there are sharks in the sea, let it only be one far out particular spot, so that 'if' we can find it, then bodies won't eventually wash up on shore, they'll be gone forever, eaten by the sharks.
